A rationale should explain the decision
Useful review identifies why the keyed response is safest, which cue supports it and why each distractor is delayed, unrelated or potentially harmful. Memorising a letter does not prepare a learner for a differently worded scenario.

Build a mistake notebook automatically

- Name the cue you missed.
- State the priority concern in one sentence.
- Write the safest first action.
- Explain why the chosen distractor was lower priority.
- Revisit the concept after a delay.
Use spaced review without memorising wording
Reattempting the same item immediately can measure recognition rather than learning. Review the principle, practise another question on the same topic and return to the mistake after time has passed.
The goal is to transfer the decision rule to a new patient, setting or response format.
