Interpret the client, not an isolated number
NCLEX questions rarely reward recalling a number without context. First identify what the test measures, then compare the result with the supplied reference range, the client’s baseline, current symptoms, medications and recent treatment. Reference intervals can vary slightly by laboratory, method and population.
A rapidly changing result may be more concerning than a stable chronic abnormality. Before acting, ask whether the specimen could be contaminated, diluted or collected at the wrong time—but never delay emergency care when the client is unstable.
Electrolytes and glucose: connect values to immediate risk
Potassium abnormalities can disturb cardiac conduction and muscle function. Combine the result with ECG changes, weakness, renal function and medications that raise or lower potassium. Severe symptoms or rhythm changes require prompt escalation and monitoring.
Sodium changes primarily affect neurological status and fluid balance. A rapid change, seizure, marked confusion or reduced consciousness is more urgent than a mild asymptomatic deviation. Glucose results should be paired with mental status, hydration, ketones when relevant and the ability to swallow safely.

- Calcium and magnesium: think neuromuscular function and rhythm.
- Glucose: think immediate neuroglycopenia or metabolic decompensation.
CBC, coagulation and renal trends
Haemoglobin and haematocrit help evaluate oxygen-carrying capacity and blood loss, but acute haemorrhage can be clinically apparent before a laboratory decline. Prioritise hypotension, tachycardia, increasing drainage, pallor, reduced urine output and altered mentation.
Platelets and coagulation studies matter most when paired with active bleeding, anticoagulant therapy or an invasive procedure. Creatinine, estimated filtration and urine output help show renal function; a falling urine output can signal declining perfusion before the next blood result is available.
A safe NCLEX lab-question sequence
Read the question’s reference range, identify the largest safety threat, assess for matching symptoms and determine whether the nurse should monitor, verify, hold a related treatment under the order, or escalate. Avoid treating a laboratory value independently without considering the client.
During review, write a short connection rather than a list of numbers: abnormal value → likely clinical effect → assessment cue → safest nursing response. This makes the knowledge usable across priority, pharmacology and NGN cases.

- Link the result to symptoms and medications.
- Prioritise airway, rhythm, perfusion, bleeding and neurological change.
- Verify unexpected results without delaying urgent stabilisation.
- Reassess after the intervention.
